This policy explains what information REVIVE Family Chiropractic(“we,” “our”) collects through okrevive.com, how we use it, and the choices you have. It covers this website only. Information you share as a patient inside the clinic is protected separately under our healthcare privacy practices.

What information do we collect?

When you submit the contact form, we collect what you type: your name, phone number, email address if provided, the reason for your visit, and your message. The form also records basic marketing attribution when present, such as which ad campaign or referring site brought you here. We use Formspree, a form delivery service, to receive these submissions by email.

When analytics are active, Google Analytics 4 collects standard usage data such as pages viewed, approximate location at the city level, device type, and how you arrived at the site. How do we use it?

Form submissions are used for one purpose: contacting you about scheduling the visit you asked about. Analytics data is used to improve the website. We do not sell your information, share it with data brokers, or use it for third-party advertising lists.

Who else processes it?

Three service providers touch website data on our behalf: Formspree (form delivery), Google (analytics), and Vercel (website hosting). Each processes data under its own privacy terms, and none is authorized to use your information for purposes beyond providing the service.

How long do we keep it?

Form submissions are kept as long as needed to follow up on your request and maintain our scheduling records. Analytics data is retained on the default Google Analytics schedule. You may ask us to delete a form submission at any time.

What are your choices?

You can call instead of using the form at any time: (405) 708-6300. You can block analytics with browser settings or extensions and the site will work normally. To ask what we hold about you, request a correction, or request deletion, email info@okrevive.com or call the office.

Children

This website is intended for adults. We do not knowingly collect information from children online; information about pediatric patients is shared with us by their parents or guardians, in the clinic.

Changes to this policy

If this policy changes, the new version is posted here with an updated effective date. Material changes will be noted plainly at the top of the page.
